Rehabilitation Service

Horizons’ rehabilitation centre was purpose built to provide assessment and therapeutic rehabilitation for people with physical disabilities, aged between 16 and 65. Often these individuals can have complex needs which require input from a number of different therapists.

The aim of the Centre is to meet the needs of the person as well as their family and carers. Time-limited rehabilitation programmes are provided by the multi disciplinary team who work together to ensure the individuals achieve their goals and gain maximum independence within the community. Horizons covers the Grampian Health Board Area, which includes Aberdeen City, Aberdeenshire and Moray.

People who are referred are either living in the community or about to be discharged from hospital. Referrals to the Centre are accepted from professionals in Healthcare and Social Work (referral must be agreed by the patient's).
