Reablement Facilitator

Job Summary

Throughout Bon Accord Care you will lead a high standard of person centred care and support for employees to support them to remain at or return to the workplace. You will lead and develop a service to promote good outcomes for your customer.

Responsibilities and Duties

The post holder will be expected to:

  • Assess competence and the embedding of a reablement ethos and best practice, across the fields of enablement rehabilitation and vocational rehabilitation
  • Assessment and provision of minor adaptations which support staff to remain at or return to work
  • Educate and promote a preventative and self-management approach to staff in order to maintain health and well-being
  • Assess the functional, psychological and emotional needs of staff and how this impacts on their work
  • Promoting recovery and overcoming barriers to participation in work through assessment and intervention
  • Liaising and working in partnership with a range of agencies and stakeholders
  • Generally promote an ethos of wellbeing across the whole organisation as part of the ‘Wellbeing at Work’ team

Qualifications and Skills

The post holder needs to be able to demonstrate an understanding or experience of:

  • The Health and Social Care Sector and the principles of providing appropriate support
  • Enablement and rehabilitation principles across a range of settings
  • Current relevant legislation and best practice
  • Assessments, reviews and support planning to ensure person centred outcomes
  • Organisational and Planning Skills
  • Office procedures including administration and using computer packages
  • The needs of our customers who require care and support; specifically recognising their need for dignity, respect and promotion of independence and wellbeing
  • The range of services and resources available in order to improve outcomes for service users
  • The post holder needs to hold as a minimum:
  • Professional qualification in Occupational Therapy, Nursing or Physiotherapy
  • Be HCPC registered
  • PDA in supervision or willingness to work towards as role required
  • Registration with relevant professional body
  • PVG registration

Job Type: Part-time 18 hours per week

Salary: £31,130.00 to £35,575.00 /year
