Over the past two years, Aberdeen City Health Care Partnership has been working hard to deliver improvements to the services that provide care for people living with Frailty. This involved major changes to how the service delivered. In redesigning the Frailty Pathway, staff moved from the hospital to community teams (Hospital at Home; Rosewell House; and community teams), to provide more care for older people closer to their homes and in their homes.
